Tattoo + Epidural Anesthesia: Separating Myth From Medical Fact
Deel
Can you get an epidural through a tattoo? We review what anesthesiologists and the literature actually say.
A persistent belief holds that you can't get an epidural (spinal/neuraxial anesthesia) through a tattooed lower back. The medical literature tells a more reassuring story.
The 'myth' review
A 2020 review titled 'Rise and fall of a myth' looked back over 18 years of literature and found no confirmed complication reported from performing epidural analgesia through a lumbar tattoo [1].
That said, reviews note tattoos can cause infections, allergic reactions and skin lesions, and that organic pigments could theoretically cause neurotoxicity if introduced into the spinal canal - so clinicians may adjust technique [2]. The practical consensus is to avoid pushing pigmented skin cores into the spinal space ('tissue coring') [3].
Potential tattoo-related complications noted in the medical literature
| Complication | Source / context |
|---|---|
| Local infection | Pain Physician 2025 narrative review Yang H et al. (2025) |
| Allergic / hypersensitivity reaction | Pain Physician 2025; Cureus 2023 case Yang H et al. (2025),Fateha MJ et al. (2023) |
| Skin lesion / granuloma | Pain Physician 2025 review Yang H et al. (2025) |
| Theoretical neurotoxicity if pigment enters spinal canal | Pain Physician 2025; Virag 2015 Yang H et al. (2025),Virag I et al. (2015) |
| Systemic epinephrine toxicity (case) | Cureus 2023 Fateha MJ et al. (2023) |
Multiple obstetric reviews reach similar cautious-but-reassuring conclusions for laboring patients with lumbar tattoos [4][5][6].
